Pet Travel Question: Dog Travel to Brazil and the United States

I'm going to Brazil for vacation for five weeks. I know that to depart from the US I have to have the rabies vaccination and the International Health Certificate endorsed by the USDA, however, I want to double check what I need to return to the US (rabies, health certificate, etc.).
