Pet Travel Question: Importing Pets to Azerbaijan

I am going to be flying my 70 lb dog on a Lufthansa flight from Houston, changing planes in Frankfurt, Germany, then going on to Baku, Azerbaijan. Can you please help me with what EXACT paperwork I will need for the entire flight? I keep reading many different requirements from various sites, and they seem to be different. Are all the travel forms the same for travel? Any help you would give is greatly appreciated. I'm planning on leaving at the end of January.
